Catherine Womack, M.D.
President, Chief Medical Officer

Cathy Womack 034Dr. Catherine Womack received her Doctorate of Medicine from the University of Arkansas for Medical Sciences in 1995 and completed her Internal Medicine Residency at Baylor College of Medicine in 1998. Dr. Womack is a Diplomat of the American Board of Internal Medicine (1998-present) and the American College of Physicians with 20 years of clinical experience. Her clinical interests are focused on the treatment of complex medical conditions such as diabetes, heart disease and hypertension. She has been recognized by NCQA for her excellence in Diabetes Care since 2010. As Chief Medical Officer, she currently directs the delivery of medical care for the entire IMWell Health organization. Her duties include oversight/training of medical staff, quality assurance and coordination of employer wellness programs. Dr. Womack has addressed several regional and national conferences regarding the value of quality primary care.

Since 2004, Dr. Womack has served as Medical Liaison for the Employers’ Health Coalition in Arkansas. In 2004, IMWell Health was incorporated to assist self-insured companies address their rising healthcare costs due to poor quality and coordination of primary care. IMWell Health is a leader in the development of on-site and near-site medical clinics which provide convenient high quality health care. Her additional duties include Medical Director for the Student Health Clinic and EMT training program at the University of Arkansas at Fort Smith. Dr. Womack is also a member of UAFS Title IX Advisory Committee.

She lives in Fayetteville, Arkansas, with her husband and two daughters.

 

Jospeh Kradel, M.D.

Dr. Kradel received his undergraduate degree from Stanford University and graduated from the University of Arkansas for Medical Science with his medical degree. He has practiced medicine in primary care and emergency room settings. Dr. Kradel also has extensive experience in the management of chronic diseases.

Terri Lewelling, M.D.

Dr. Lewelling is board certified in family practice with professional experience in inpatient and outpatient family practice with special interest in women’s health, pediatrics, dermatology, migraines and minor procedures. She obtained a medical degree, with summa cum laude distinction, from The University of Oklahoma College of Medicine. Dr. Lewelling received her postgraduate training at Great Plains Family Practice Residency at Baptist and Deaconess Hospitals in Oklahoma City. Prior to medical school, she received a Bachelors of Science degree from Oklahoma Christian University.

Scott Mays, M.D.

Dr. Scott Mays is a graduate of the University of Texas Medical School in Houston, Texas. He is certified by the American Board of Family Physicians and has 27 years of experience in family practice. Dr. Mays is a member of the American Academy of Family Physicians and the Oklahoma Academy of Family Physicians. Dr. Mays’ interests include medical missions in many parts of the world.

Rodney McDonald, M.D.

Dr. McDonald is the medical director for the Fort Smith Central multi-employer clinic. He is board certified in family practice and has 10 years of practice experience. He graduated from medical school at the University of Arkansas for Medical Sciences and received a Bachelor of Science in Neuroscience from Texas Christian University.

Shawn Miller, M.D.

Dr. Shawn Miller received her undergraduate degree from Oklahoma State University and graduated from the University of Oklahoma Medical School in 1996. She has been board certified since 1999 in family medicine. She has provided outpatient as well as inpatient clinical care for patients in the Fort Smith area since 1999. She has also been in private practice and provided family medicine with obstetrics.

Thomas Schooley, D.O.

Dr. Thomas Schooley received his undergraduate degree from Oklahoma State University and graduated from the Oklahoma College of Osteopathic Medicine and Surgery in 1987. He has been board certified in family practice since 1996. He has worked in a variety of areas including private practice, group practice, urgent care and for a Native American community hospital.

Kanan Shaw, M.D.

Dr. Shah has 14 years of experience in health care. She is a graduate of the University of Kansas Medical Center. She specializes in women’s and adolescent health in addition to primary care.

Daniel Studdard, D.O.

Dr. Daniel Studdard has 24 years of experience as a family practice physician. He is a graduate of Oklahoma State University of Osteopathic Medicine. He is also an Adjunct Clinical Assistant Professor in the Family Practice Department of Oklahoma State University.

Teresa Bartlett, APN

Teresa Bartlett has over 30 years of experience in primary care. She attended the University of Arkansas Medical Sciences graduating with an RN, BSN, and RNP. She received her MSN and APN in 2004 from the University of Central Arkansas. Teresa is married with one daughter.

Brittany Bogan, PA-C

Brittany Bogan has worked in the health care profession as medical assistant and microbiologist prior to receiving her P.A. degree. Brittany graduated from Harding University with an MS in Physician Assistant Studies and received a Bachelor of Science degree in Biology from Arkansas Tech.

Bilha Candelaria, APN

Bilha Candelaria has nine years of experience in family practice. She graduated with her Doctor of Nursing Practice degree from the University of Texas at El Paso, where also has served as a clinical instructor. She specializes in pediatrics and initiated the beginning of employee health based clinics for Ysleta Independent School District with over 6,000 employees. Bilha is also a Health Leader/Minister at her church. She believes in healing as a holistic approach–helping patients not only physically but offering a smile may be all they need to heal their soul and physical being.

Carol Cook ANP, MNSc, FNP

Carol Cook is a National Board Certified Family Nurse Practitioner with 14 years of advanced practice experience and a total of 33 years of nursing experience. Carol has had a wide range of experience in the primary care field, focusing primarily on preventative health care as well as acute and chronic illnesses. Carol has served on the Board of Directors for the American Heart Association and Arkansas Nurses Association. She is also a volunteer for the Fort Smith Little Theatre.

Lisa Eckhardt, PA

Lisa Eckhardt has 25 years of experience in the medical field. She is a graduate of Trident University International and Wichita State University. She has practiced in both clinic and hospital settings.

Dana Hale, APN, MSN

Dana Hale has 13 years of experience as an Advance Practice Nurse in primary care and women’s health. She received her Masters degree as a Clinical Nurse Specialist from the University of Central Arkansas. She completed her Nurse Practitioner degree from the University of Texas Southwestern Medical Center and George Washington University and is board certified in Women’s Health and Family Practice. Dana is a member of Sigma Theta Tau and the Honor Society of Nursing.

Erin Holleman, APN

Erin graduated from the University of Arkansas for Medical Sciences. Her seven years of work experience has given her a comprehensive background in urgent care and burn wound care. Erin is a member of Sigma Theta Tau International Society of Nursing and has volunteered at Camp Sunshine, a summer camp for children burn survivors, for the past 7 years.

Traci Howe, APN

Traci Howe, a native of West Monroe, Louisiana, received her advanced nurse practitioner degree from the University of Arkansas Medical Sciences. She is Board Certified in Family Nurse Practitioner. Traci has seven years of extensive experience in Internal Medicine and Cardiology.

Jacqueline Phillips, APN, MNSc

Jacqueline has over 14 years of nursing experience in the areas of outpatient primary care, inpatient geriatric care and teaching. She received her Masters in Nursing Science from the University of Arkansas for Medical Sciences (UAMS) and a Bachelor of Science in Nursing from UAMS as well. She is also a member of Sigma Theta Tau Honor Society of Nursing and received the Outstanding Future Leader Award in graduate school.

Delara Pollman, APRN, CNP

Delara Pollman is a National Board Certified Family Nurse Practitioner, who completed her undergraduate studies at the University of Oklahoma with a Bachelor of Science in Nursing.  She continued her education, receiving a Master’s of Science in Nursing from the University of South Alabama.  She is a member of the American Academy of Nurse Practitioners and the Nurse Practitioners of Oklahoma.
Delara has over six years of experience encompassing orthopedics, neurosurgery and family care.  She has a passion in preventative medicine and loves taking care of and getting to know her patients and their families.

Jennifer Smith, APRN, CNP

Jennifer has over 10 years of experience in emergency medicine, family health, pediatrics and ICU. She enjoys building a relationship with her patients and seeing them improve their health.  She takes care of all ages from newborn to elderly and enjoys the diversity family practice has to offer. Jennifer received a Master’s of Science in Nursing as a Family Nurse Practitioner from the University of Missouri -Kansas City and a Bachelor’s of Science from Oklahoma Wesleyan University. She is board certified as family nurse practitioner by the American Academy of Nurse Practitioners. She is a member of the American Academy of Nurse Practitioners, 4-states APN and the American Nurses Association.

  • Dr. Catherine Womack, our Medical Director, is Nationally Recognized as a member of the National Committee for Quality Assurance (NCQA) Diabetes Recognition Program 2010-2017.
  • Dr. Rodney McDonald, Director of the Ft. Smith Central Multi-Employer Clinic, is the second physician at IMWell Health to earn National Recognition as a member of the NCQA Diabetes Recognition Program 2011-2014.
  • Dr. Terry Lewelling, Director of the OK Industries On-Site Clinic, is the third IMWell Health physician to be Nationally Recognized as a member of the NCQA Diabetes Recognition Program 2012-2015. Dr. Lewelling’s achievement also made her one of only five Nationally Recognized Doctor’s practicing in the state of Arkansas. Three of the five, all mentioned here, work exclusively for clients of IMWell Health.
  • Dana Hale, nurse practitioner at the WWL On-Site Clinic, is recognized as a member of the NCQA Diabetes Recognition Program 2015-2018.
  • Erin Holleman, nurse practitioner at the Hot Springs Multi-Employer Clinic, is recognized as a member of the NCQA Diabetes Recognition Program 2014-2017.
