How do you get started?

Getting an appointment at an IMWell Health clinic is hassle free. Simply call your clinic and schedule an appointment. But, before you’re your first appointment you will need to fill out or bring in the following:

• Valid photo identification – Including either a driver’s license, employment badge, passport, military ID or school identification card.
• Your company insurance card – For verification purposes.
• IMWell Health Patient Registration & Medical History Form – Which can be printed and completed in advance, or a copy will be available to you at check-in.
